Click Through Rate:- This is defined as what is the percentage of your clicks people are click out of 100. For example my site click through rate is between 18% to 25% take the average and place your click through rate(CTR) in the click through rate box.

Cost per click: - This is defined by how much you earn per click a click can be from .05 cents till $100. My average click is around .25 cents to $3 per click. Click your average cost per click in Google Adsense report and put your CPC in the box.
